Our free-standing rural health clinical endeavor to implement the patient-centered medical home healthcare delivery model. The clinics are vertically integrated have systems in place to coordinate the care of our patients under one roof. Focusing on primary care, we work with multiple provider types and functional systems to ensure we help the patient engage care across the continuum. By using scarce resources in order of priority, we ensure our patients have access to care at the most appropriate time and intervals. This means we start out with mid-level providers, specifically nurse practitioners for the evaluation and management of disease and trauma, both acute and chronic. We may use licensed clinical social workers for the evaluation and management of behavioral health disease and trauma. Our goal is to minimize our patients' needs to access care outside the clinic. Therefore, we will also partner with other stakeholders to provide telemedicine and specialist care when appropriate within our clinic.
We will endeavor to help our patients navigate the continuum of care outside our clinic as well when required for modalities such as in-patient hospitalization, specialized care, physical therapy, home health, hospice, pharmacy, durable medical equipment, and/or certain diagnostic testing.
We will endeavor to provide healthcare that is congruent with the patient's heritage and belief system. Appreciating diversity and the need for respect allows our team to cultivate an environment of trust and honest communication. Because genetics, environmental factors, family dynamics, and social morays all impact our health and well-being, it is important to recognize vulnerability within specific people groups. Having the resources to create a dynamic, cross functional, multi-modal, clinics will allow greater continuity of care which will allow for better outcomes. We share common values which endear us to one another. Love, honesty, respect, and kindness are not only moral Imperatives; they are the currency of our sacred work.
We will work to remove barriers to access healthcare in the community by liaising with local community leaders and organizations ensuring every person in the community is aware of our services, hours, location, and availability. Moreover, we will work with local schools, places of worship, and other helping organizations to reach vulnerable populations such as women, children, and seniors. From community education to health fairs, our goal is to be active in the community. We will work with schools to ensure students are prepared for education opportunities by providing medical care and screenings at their school, or even developing school-based health centers should the need exist. In certain communities, there may be language barriers. Our clinics will use contracted translators to communicate with potential patients and their families. Of course, our clinic will endeavor to employee bilingual employees, however it is important to actively participate in the community and to that extent, we will use translators when appropriate.
