Scheduling an Appointment:

   If you would like to schedule an appointment, please call our main telephone number at 1(800) 462-0742 and select the extension for your desired location or submit a request via patient portal:

•   Cottonport Family Clinic - Ext. 11

•   Mansura Family Clinic - Ext. 13

•   Simmesport Family Clinic - Ext. 12

•   Palmetto Family Clinic - Ext. 15

• Marksville Family Clinic - Ext. 14

•    When scheduling your appointment, please explain to our staff the reason for your visit. This allows us to schedule the appropriate time needed to address your health concerns.

•   Please note, appointments are for Fast – Track (create link to Fast Track Page) use ONLY.

•  New Patients are asked to fill out the appropriate forms here(create link to new patient section) and bring them to their first visit along with:

•   Insurance card(s)

•   List of medications, including dosage and how/when you take them, or bring all medications with you

•   Photo Identification

•   Social Security card

•    New patients do not need to schedule appointments, just walk – in.
