  • It is our policy to collect payments upon service, including any deductible, co – payments and self – pay payments. If your insurance has a co – payment, your insurance carrier requires that we collect it from you at the time of service.

  • We bill most large insurance carriers as a courtesy of our patients; however, we always suggest you check with your insurance company to verify that our Nurse Practitioners are participating providers in your plan.

  • Below is a list of most insurance carriers that we accept. If you have any questions about your fee or the insurance companies with which we contract, please contact our billing staff at (800) 462-0742 EXT 150.

  • BlueCross / BlueCross PPO BlueCross HMO

  • Aetna – Private and Medicaid/Healthy LA

  • United Health Care – All Carriers

  • ChampVA

  • Tricare

  • Vantage – Private and Medicare

  • Wellcare

  • Humana

  • We are not in network with Cigna. To verify if we are in network with your insurance, please contact your insurance company.

  • If the patient will not be responsible for payments of services, payment arrangements must be made with the responsible party prior to the visit.
