•  Routine checkups and laboratory testing for patients of any age

• Women’s well visit, including breast exams and pap smears
•   Cancer Screenings, including blood test for prostate abnormalities, pap smears, and referrals for colonoscopies and mammograms.
•  Cardiac screening, including laboratory testing, referral for EKG/ECG, and referrals to cardiologist
•   Annual Adult Wellness visits

Examples of chronic diseases include: 

• Hypertension (High blood pressure)
• Hyperlipidemia (Elevated cholesterol)
• Diabetes
• Renal Impairment 
• Thyroid Disorder
• Respiratory disorders, such as COPD, asthma, etc. 
• Arthritis 

Services for chronic disease management include, but are not limited to:

• Screenings for hypertension, hyperlipidemia, diabetes, thyroid disorder, etc
• Medication management such as insulin therapy and oral medications
• Routine follow-up exams, including risk factor modification
• Education and dietary resources
• Regular disease management and risk factor reduction
